Registration fee includes coffee, tea, lunches, drinks and presentation syllabus.
The Conference dinner (Thursday, June 17) is not included in the registration fee. Costs for the dinner are € 60,- per person (accompanying persons as well).
Registration
You can register by using the registration form. In the registrationform you will be asked to indicate your way of payment. There are three options:
- Credit card: the registration form generates a Fax Formv which you can print, complete and send.
- Automatic Bank Payment (for Dutch participants only): u geeft uw bankrekening op voor een automatische incasso
- Bank: remit the total amount to the SNS-bank account number: 90.85.61.180 of the Wenckebach Instituut, Hanzeplein 1 in Groningen. Quoting invoice number '0842' and your name.

We can send you an invoice on request. You can make a note 'Sending Invoice' on your registration form by 'Special requirements'.
Cancellation
Cancellation of your registration is possible before April 15, 2010. Until April 15th, 2010 the registration fee will be refunded, excluding a € 50,- administration fee. After April 15th, 2010 no refunds on registration will be made. No-shows will not be refunded.
Notification of cancellation should be made in writing (post or e-mail) to the Meeting organizers:
