Costs and registration

Registration is possible until January 27, 2012, by using the registration form at the bottom of this page. 

You can register for the symposium and the workshops. You will be confirmed as soon as you have registered.

The registration fee includes tuition for:

• the symposium
• the program
• writing material
• coffee and tea
• lunches
• ‘get-together’ party on Wednesday evening
• dinner on Thursday evening
• abstract book

Registration for the workshops on Saturday is only possible for participants for the symposium. The registration fee for the workshops is not included in the registration fee for the symposium.

There is no reduction of the participation costs if you don't join all days and/or all social events.

Reduction for representatives from several Departments of the UMCG:

Representatives from the Department of Orthodontics and Department of Oral and Maxillofacial Surgery from the UMCG are free of charge. Please include the Personnel number of the UMCG when you register.

Way of Payment

There are three options for the payment:

• Automatic Bank Payment: This option is only possible for Dutch Residents by authorizing the Wenckebach Instituut for collecting the registration fee.

• Payment by Credit card: The Registration Form generates a Fax Form which you can print, fill in and send to our address. Fax number: +31 (0)50 3611819

• Payment by Bank

Remit the total amount to the SNS-bank account number: 90.85.61.180

Quoting the Invoice Number: '0891 - name of the participant’.

An invoice will be sent on request.

Please make a note 'Sending Invoice' on your Registration Form by 'Special requirements'.

Cancellation
Cancellation of your registration is possible before January 10, 2012: full refund minus € 50,- handling fee. Cancellation after January 10, 2012: no refund. No-shows will not be refunded.

Notification of cancellation should be made in writing (post or e-mail) to the Meeting organizers:
